1. Medical Information and Disclaimer

2. Establishing Care
New patients must complete all required intake forms and provide accurate medical history. Prior medical records may be required to establish care safely. The provider will determine whether our practice is an appropriate fit for ongoing care based on clinical judgment.

3. Appointments, Cancellations, and No-Shows
Appointments are required for all primary care services. Cancellations or rescheduling typically require 24–48 hours notice. Late arrivals may be rescheduled. No-show and late cancellation fees may apply. Repeated no-shows may result in dismissal from the practice.

4. Payment, Insurance, and Billing
Patients are responsible for verifying their insurance coverage. Co-pays, deductibles, and non-covered services must be paid at the time of service. Any remaining balance after insurance processes a claim is the patient’s responsibility. Certain services may not be eligible for insurance billing and may require self-pay. There are no refunds for medical services that have been provided.

5. Prescription and Refill Policies
Medication refills require an established provider–patient relationship. Controlled substances require in-person evaluations and adherence to Texas law. Early refill requests for controlled substances will not be approved. An appointment may be required before refills are granted. Refill requests may take 48–72 hours to process. Prescriptions and refills will not be provided through voicemail, email, text message, or social media.

6. Lab Work, Imaging, and Follow-Up
Patients are responsible for completing labs or diagnostic studies as ordered. Follow-up appointments may be required to review results or adjust treatment plans. Some labs require preparation such as fasting.

7. Telehealth Services
Telemedicine visits are offered when clinically appropriate. Some conditions require in-person evaluation to ensure safety and compliance with Texas Medical Board regulations. Patients must be physically located in Texas during telehealth visits. Providers may convert a telehealth visit to an in-person visit if necessary.

8. Communication and Electronic Messaging
Phone calls, portal messages, and emails are not intended for urgent issues. Electronic communication may not be secure unless sent through the patient portal. Medical advice will not be provided through social media, text messages, or unencrypted email. Response times may vary. Sensitive or complex medical issues require an appointment.

9. HIPAA, Privacy, and Medical Records

3. Multiple Sessions
